Introduction

 

Starting from January 2013, the Food and Environmental Hygiene Department (FEHD) will accept online filing of applications with the use of electronic application form(e-Form) through the Internet.

 

Recommended System Requirements

 

To use the e-Form, the recommended system requirements are listed as follows:

i) Hardware Requirements
  1 CPU: Pentium 4 or above
  2 RAM: 1 GB or above
  3 Display Resolution: 1024 x 768
     
ii) Software Requirements
  1 Microsoft Windows running 2003/XP/Vista/7 or above, Fedora Linux Core 7 or above, Apple Mac OS X 10.5 or above
  2 Internet Explorer 6.0, Firefox 2.0.x or above, Safari 4.0 or above and Chrome
  3 Java Runtime 1.6 or above
  4 Hong Kong Supplementary Character Set – 2001 (for Windows XP or lower version)
     
iii) Internet Connection Bandwidth:
  512 Kbps or above

Requirement of Proposed layout plan(s)

 

Under the new arrangement of electronic application (e-app), applicants seeking any of the licences and permits mentioned above may fill in the relevant electronic application form and submit it to FEHD together with the proposed layout plan(s), where required, in electronic format. The submitted layout plans should meet the following criteria :

 

(a)              they should be in “PDF” format;

(b)     the total file size should not exceed MB;

(c)              they should be drawn to the metric scale of not less than 1:100; and

(d)             the paper size of each plan should not be larger than A3.

 

Arrangement of FEHB 192, Declaration Form

 

Except for food business premises in Government properties, Housing Authority’s properties or the properties divested to The Link Management Limited, the applicant is required to attach an electronic copy of a declaration (FEHB 192), and the original copy shall be submitted (in person or by mail) to this department declaring that the operation of the food business at the premises under application is in compliance with Government lease conditions, otherwise this department is unable to process the application.

As the provision of the Digital Signature is one of the mandatory requirements in submitting this e-form, should you fail to submit the Digital Signature or do not have the e-certificate to provide the Digital Signature, you may print out the form and sign on the hardcopy and send to respective Licensing Offices by mail or in person.

 

@ For eFEHB 105, digital signature is ONLY required when submitting application of Undertaker’s Licence.

 

Additional Email Address (if any)

 

For each application, applicants must submit one email address for future correspondence with FEHD by email. If the applicant would like to provide more than one email address, he/she could provide it at the field named “Additional Email Address (if any)”. You may wish to note that these two email addresses will receive all the emails sent by the FEHD simultaneously.
