Applicant has to:

  1. produce his identity card and the original and copy of the Exhumation Permit issued by FEHD;
    1. a copy of his or her own identity card;
    2. a copy of the identity card of the deceased;
    3. original and copy of the Permit to Take Away Cremated Ashes of the deceased;
    4. for application to deposit ashes of more than one deceased person, documentary proof of the relationship between the deceased is required. If such proof is not available, the applicant shall take an oath at the Cemeteries and Crematoria Office of the Food and Environmental Hygiene Department (FEHD) in Hung Hom or Happy Valley;
    5. for application for niches on outlying islands, the original and a copy of letter from the relevant islands rural committee and a statutory declaration made by the legal representative or next of kin of the first decreased person are required to certify the eligibility of the deceased;
  2. complete an application form;
  3. submit in person the completed application form, together with the originals and copies of the above documents and a copy of the applicant's identity card, to the Cemeteries and Crematoria Offices of FEHD at 6 Cheong Hang Road, Hung Hom, Kowloon or 1J Wong Nai Chung Road, Hong Kong [for submission by post, only copies of the Permit to Take Away Cremated Ashes, the applicant's identity card and the original letter from the islands rural committee (for application for niches on outlying islands only) are required];
    [Note: after completing the procedures, the application will be processed and a written reply will be sent to the applicant in about three working days.]
  4. upon receipt of the written reply and demand note, pay the fees within one month at the designated shroff office and the niche will be allocated;
  5. after payment, arrange for the deposit of ashes at the office of the columbarium with the mason.
