Allocation of Niches in the Columbarium of Cheung Chau Crematorium

The remaining stock (about 38 large and 912 standard niches), after the closing date of application by balloting, of the additional 1 026 niches at Cheung Chau Columbarium constructed by the Food and Environmental Hygiene Department (FEHD) will be open for registration for application on a first-come-first-served basis. These niches are allocated to eligible Cheung Chau residents only. The deceased must be certified by the Cheung Chau Rural Committee as a local indigenous villager, or a bona fide resident living on Cheung Chau continuously for not less than 10 years, or an offspring of local residents. All applications will be processed Details are as follows:

Period of Application
From 9:00 a.m. on 6 June 2007 (Wednesday) until all the niches are sold.

Application Form
"Application for a Niche/Placing Additional Urn(s)" Form (FEHB 136) can be obtained in person from the FEHD offices at Room 111, 1/F, Cheung Chau Market Complex, 2 Tai Hing Tai Road, Cheung Chau or 1/F, Cheung Sha Wan Government Offices, 303 Cheung Sha Wan Road, Sham Shui Po, Kowloon, obtained by fax (2981 4277 / 2765 6402) or downloaded from the FEHD website (www.fehd.gov.hk).

Submission of Form
Applicants must fill out the application forms and submit them together with the following documents, in person or through an authorized agent, to the FEHD office at Room 111, 1/F, Cheung Chau Market Complex, 2 Tai Hing Tai Road, Cheung Chau during office hours of the above application period. (An applicant or an authorized agent is allowed to hand in one application in every submission.)

(1) A copy of the applicant's identity card.

(2) A copy of the deceased's identity card.

(3) A copy of the Permit to Take Away Cremated Ashes or Certificate of Cremation of the deceased.

(4) A certificate issued by Cheung Chau Rural Committee.

(5) For placing the ashes of more than one deceased person, the applicant should submit a copy of the documentary proof of the relationship between the deceased.

Processing of Applications

(1) FEHD will initially vet the application forms and related documents before making registration with applicants on a first-come-first-served basis. The applicants will be informed in writing by registered post of the appointment date and time for selection of niches at Room 111, 1/F, Cheung Chau Market Complex, 2 Tai Hing Tai Road, Cheung Chau in accordance with the priority number of the registration.

(2) Whether an applicant can be allocated a new niche depends on whether the new niches for allocation have been sold out on the day and time of his/her appointment.

Points to note

(1) The current charges for one standard niche (can accommodate 2 urns) and placing of a memorial plaque are $3,000 and $295 respectively.

(2) The current charges for one large niche (can accommodate 4 urns) and placing of a memorial plaque are $4,000 and $295 respectively.

(3) FEHD has never commissioned any organizations/persons to assist/act for applicants in their applications. No organization or person shall enjoy any convenience or privileges over any other member of the public.
